If your plan is not shown here, please contact your insurance carrier to ask who they cover for lactation services. 

We are able to provide superbills for you to seek reimbursement for services to all self-pay clients.  This does not guarantee that your carrier will reimburse you.

At this time, while we are in-network with Medicaid CHP+ it requires a referral - please have your provider put your referral into the Colorado Medicaid System

Home Visits incur a home visit fee of $30 and $60 if in our extended travel area; this fee is not covered by your insurance coverage and is due at the time of service.
